How to Apply for a West Virginia Child Labor Work Permit
If you’re 14 or 15 years old and want to work, follow these steps to get a Work Permit:
Step 1: Employer Completes Section A
- Fill in the minor’s name, job duties, start date, and business details.
- Sign and confirm compliance with state work hour limits.
Step 2: Parent/Guardian Completes Section B
- Provide the minor’s name, birthdate, birthplace, and attach a certified birth certificate.
- Sign and include contact information.
Step 3: School Official Completes Section C
- A principal, registrar, or superintendent verifies the minor is enrolled, then signs and dates the form.
Step 4: County Official Approves the Permit
- A superintendent or authorized official reviews the form and either approves, modifies, or rejects it.
Step 5: Submit Form to the Records Department
- Once all sections are completed, submit the form to the Records Department at:
200 Elizabeth Street, Charleston, WV 25311 or at [email protected].
- A Records Department staff member will review the documents and submit them to the West Virginia Division of Labor.
